Please bring the following documents with you to your registration appointment:

• Photo ID
• Proof of Residency
• Original Birth Certificate
• Immunization Records or Utah Exemption Form
• Previous School Records
• Proof of Guardianship
• Residency Affidavit

School Choice Applications

Families interested in schools outside their attendance area must submit a school choice application to be considered.

School Choice Timeline

• October
  Schools are declared as open or closed at the October school board meeting
• November 15 Web Posting:
  • Building capacity, utilization factor, and functional capacity
  • Previous year’s number of applications, acceptances, and transfers
• November 15–1st Friday in February
  Early Open Enrollment Applications accepted
• March 1
  Late Open Enrollment Applications accepted
• March 31
  Notification of acceptance/denial of Early Open Enrollment Applications
• April Web Posting:
  • April enrollment
• August (the Friday before school begins)
  Notification of acceptance/denial of Late Open Enrollment Applications
